New patients will be offered appointments within two working days of contacting our office. Emergency referrals will be honored upon the request of a physician. All intake forms are available at our website (www.tuckahoept.com) or they can be faxed if desired. Hours are Monday-Friday, 7:45 A.M. – 5-30 P.M. by appointment.
